- Apply at healthearizonaplus.gov or call 1-855-432-7587. Save your confirmation number.
- Call the interview line at 855-777-8590, Monday to Friday, 7 AM to 3 PM, to do your interview.
- Open every letter from DES. Keep your address and phone number up to date.
